Admission Requirements

  • Freshmen
  1. Two (2) long folders with fasteners
  2. Original and photocopy of Report Card (Form 138)
  3. Two (2) pcs. photocopy of NSO Birth Certificate
  4. Original and photocopy of Certificate of Good Moral Character signed by the Principal or Guidance Counselor from previous school
  5. Copy of Entrance Exam Result
  6. Copy of Student’s Permanent Record (Form 137) to be submitted one (1) month after start of classes
  • Transferees
  1. Two (2) long folders with fasteners
  2. Original and photocopy of Honorable Dismissal
  3. Original and photocopy of copy of grades
  4. Two (2) pcs. photocopy of NSO Birth Certificate
  5. Original and photocopy of Certificate of Good Moral Character signed by the Principal or Guidance Counselor from previous school
  6. Copy of Entrance Exam Result
  7. Copy of Official Transcript of Record with remarks “Copy for Olivarez College” to be submitted one (1) month after start of classes
  • Foreign Students
  1. Academic documents authenticated by the Philippine Embassy and/or Consulate official located in the student’s country of origin (with English translation, if written in foreign language)
  2. Student’s Personal History Statement
  3. Photocopy of student’s passport showing its biopage, latest arrival and authorized stay in the Philippines
  4. Original and photocopy of Medical Certificate issued by Bureau of Quarantine
  5. Original and photocopy of birth certificate
  6. Photocopy of Marriage Certificate (for female, if married)
  7. Three (3) pcs. 2×2 ID picture
  8. Three (3) pcs. 1×1 ID picture
  9. Two (2) pcs. long folder with fasteners
  • Cross-Enrollees
  1. Cross Registration Permit from home school
